Designated Coordinator

Hours: Monday - Friday, 7:30 to 4:00 pm
Wage: $26.50 per hour

Responsibilities: Provides case management, job training, and vocational support to individuals with disabilities receiving services through the Waivered Services Department. Ensures all services meet DHS 245D Home and Community-Based requirements for Day Support, Employment Exploration, Employment Development, Employment Support, and Pre-Vocational Services. Supports each person in achieving their employment goals by coordinating services, delivering vocational programming, and building strong relationships with service providers and employers.

Qualifications:

  • Bachelor’s Degree or equivalent experience as outlined in MN 245D license requirements.
  • Valid Driver’s License and insurable driving record along with ability to pass Criminal Background Check.
  • Must be able to fluently read, write and speak in English language and communicate effectively and efficiently.
  • Organized, strong work ethic, ability to multi-task and prioritize.
